One certified check or money order payable to the Kentucky State Treasurer in the amount of $300.
Re-license will be based on the tonnage from the previous year, $300 plus $100 for each additional 100,000 tons up to $1,500 maximum.

Two notarized up-to-date maps of the mine certified by an engineer licensed in Kentucky.
The maps must show a five-year projection of the mine.
One surface
map transmittal letter
must accompany the maps.
Workers' Compensation coverage
(Our office will verify the coverage through the Department of Labor in Frankfort, Ky.).
Approval for a mine license to be issued from the Department of Revenue in Frankfort, Ky.
